New Patient Paperwork

We are eager to get you connected to your new therapist!

Getting you to your new paperwork begins with these steps:

1. Please contact us by calling 205-807-9874 or use the email form at the bottom of this page.
2. Our Solutions Advocates will contact you as soon as possible in order to start you on your journey to better mental health.
3. After verifying your insurance details and method of payment, they will schedule your first therapy session and email a link to your confidential portal. (Make sure you enter the portal within 24 hours.)
4. When you click on the link to your portal, follow the instructions that will take you to the online paperwork that needs to be completed before your first therapy session
Please Remember:

Your therapist wants to provide you with expert mental health services and a treatment plan that is tailored specifically to your needs. Completing your paperwork before your first session will give your therapist an opportunity to best prepare for your work together.

Don’t worry – the Intake Questionnaire is the only detailed document! All the other documents are short and simply require your signature. Most of the therapists at BATT will then review your intake information with you during the first session to make sure they have a complete picture of you and how therapy can help. The portal is easy to navigate and helps us save trees!
